Request Services for a Home Inspection

Below are steps to complete an application for temporary re-connection service for a home inspection.

Before clicking the application link below, which will take you to the application form in another page, please read all the instructions carefully, so that you don't miss a step and delay the account opening process. Below you will find as list of all required documents, as well as the steps for submitting the application.

Application Instructions

Required documents:

  • Completed Application for Service.
  • Signed Acknowledgment of Release of Liability “Waiver."
  • Driver’s license for primary account holder. (scan or photocopy).
  • Legal proof of ownership or other interest in the property.
  • If an authorized representative applies, include proof of legal representation (POA, court documents, etc.).
  • Payment fee: $25.00 non-refundable temporary re-connection fee - check or money order (no fee).
  • If paying with credit card a 3.10% processing fee will apply. 

Application Steps:

Step 2.Attach documents

Attach HUD/Warranty Deed and any POA/Estate legal documents.

Step 3.Submit application & fee

Submit application, waiver, required documents to: SLWSD Customer Service

Step 4.Pay the Deposit

Call SLWSD Customer Service to pay the $25.00 non-refundable temporary re-connection.
